Our Clinical Process

How We Manage Your Health Journey

To ensure the highest standards of safety and clinical accuracy, our nursing services follow a structured pathway:

  1. Initial Nursing Assessment: A Registered Nurse visits your home to conduct a holistic assessment of your medical history, current health status, and goals.

  2. Care Plan Development: In consultation with your doctors, we co-design a detailed Clinical Care Plan that outlines specific nursing interventions and schedules.

  3. Competent Delivery: Care is delivered directly by a qualified nurse, or delegated to a strictly trained, competency-assessed support worker under direct nursing supervision.

  4. Regular Review & Reporting: We continually evaluate your health progress, updating your care plan as your medical needs change, and provide detailed clinical reports for your NDIS plan reviews.

NDIS Categories & Funding

Navigating Your NDIS Nursing Funding

Community Nursing is a clinical service funded under the Core Supports or Capacity Building budgets of your NDIS plan. It is primarily accessed via:

  1. Delivery of Health Supports / Community Nursing Care (Registration Group 0114)

Please note: NDIS clinical nursing is designed to address health conditions directly related to a participant’s primary disability. Our intake team can assist you or your Support Coordinator in verifying how your funding lines apply.
